To enroll or re-enroll in 4-H please go to the 4hOnline website https://california.4honline.com Complete 4hOnline enrollment and give the Medical Release and Health History forms to your Community 4-H Club Leader. The Volunteer Confidential Self Disclosure form can be given to your Community 4-H Club Leader or sent to the UCCE San Mateo County 4-H Office.

Please only print these out for families with limited to no internet access. The majority of enrollment should be completed through 4hOnline to conserve resources.

4-H Youth Development Program Fees
4-H Youth Development Program Fees from individual members are paid to the 4-H Club that you join. The 4-H Club may have specific fees for administrative purposes and supplies. The enrollment fee per club varies. Please contact the 4-H Club Community Leader for the total fee. From the fee paid, the club then pays the County 4-H Office $25 per 4-H member and $6 per volunteer adult. The County 4-H Office pays the State 4-H Office $12 per 4-H member and $6 per volunteer adult to cover limited accident/illness insurance, statewide program and curriculum costs for youth, and accident sickness insurance and liability pool insurance for 4-H adult volunteers. The County 4-H YDP Fees provide free or minimal fees for countywide events, activities, and awards.
